COVID-19 Vaccine Requirement
All students, faculty, and staff must be vaccinated against the COVID-19 virus before returning for the 2022 - 2023 academic year. The requirement was put in place by the President and Senior Administration in consultation with the University Action and Response Team based on the following:
- COVID vaccines (Moderna and Pfizer) have full FDA authorization and have been proven to be safe and effective.
- Vaccine requirements achieve high immunization coverage and protect communities from infection rates and the severity of the disease. The St. Thomas community vaccination rate is over 90%.
- Religious and medical exemptions are permitted.
At this time, St. Thomas is not requiring COVID-19 boosters to live, work or attend classes on campus, although the university will continue monitoring the overall situation and make policy adjustments as needed.
If you do not get the COVID-19 vaccination or apply for an exemption, a hold will be placed on your account, and you will not be able to register for classes

Documentation & Exemption Instructions
All students, faculty and staff must be vaccinated against the COVID-19 virus before returning for the Spring 2022 term.
If you do not get the COVID-19 vaccination or apply for an exemption, a hold will be placed on your account and you will not be able to register for classes.
St. Thomas will work with students who have been vaccinated abroad as well as international students who do not have access to vaccines approved in the US. See more information below.
The vaccine requirement will remain in place for the 2022-2023 academic year.
Action Required:
All students without a vaccination record or with a Fall 2021 religious or conscientious exemption must take one of the following actions by January 18, 2022:
- Submit proof of vaccination - Upload a copy of your vaccination card to myHealthPortal.
- Submit a medical exemption - Complete the medical exemption form with your licensed clinician and upload it to myHealthPortal.
- Submit a religious exemption - Sign the religious exemption form in front of an off-campus notary and upload it to myHealthPortal.
Exemptions forms may also be faxed to the Center for Well-Being at (651) 962-6751.
Students who are not fully vaccinated must:
- Comply with current and future campus mitigation efforts
- Isolate for 10 days if diagnosed with COVID-19
- Quarantine for 10 days if there is close contact with someone diagnosed with COVID-19
Check to see if the Center for Well-Being has a record of your COVID vaccination.
- Login to myHealthPortal
- Click on My Profile
- Click Immunization History in the dropdown
- See if your COVID-19 vaccinations are listed (2 shots for Pfizer or Moderna; 1 shot for Johnson & Johnson). If they are, no further action is needed.
If your vaccination is not listed and you have been vaccinated:
- Click Document Upload
- Click Document Type
- Select COVID-19 Proof of Vaccination
- Take a photo of your CDC vaccination card
- Upload the photo to the portal and click save
Students who are unable to get the COVID-19 vaccination due to medical reasons may apply for a medical exemption. To apply for this exemption:
- Print the COVID-19 Medical Exemption Form
- Have your medical provider sign the form
- Upload a photo of the completed form through myHealthPortal:
- Select Document Upload from the menu
- Select 'Vaccination Exemption' as the Document Type
- Select Browse then 'Photo Library' or 'Choose from Gallery'
- Select the photo of your exemption form
- Select Save and wait until you see the 'Upload Successful!' message
The form may also be faxed to the Center for Well-Being at (651) 962-6751.
Students who are unable to get the COVID-19 vaccination due to sincerely held religious beliefs, practices and/or observances may apply for a religious exemption. To apply for this exemption:
- Print the COVID-19 Religious Exemption Form
- Sign the form in front of an off-campus notary.
- Upload a photo of the completed form through myHealthPortal:
- Select Document Upload from the menu
- Select 'Vaccination Exemption' as the Document Type
- Select Browse then 'Photo Library' or 'Choose from Gallery'
- Select the photo of your exemption form
- Select Save and wait until you see the 'Upload Successful!' message
The form may also be faxed to the Center for Well-Being at (651) 962-6751.
The Minnesota Department of Health does not provide replacement cards. Instead, Minnesota residents
can request a copy of their vaccination record.
Contact your specific state health department if you’re a resident of another state.
Students arriving from an international location that does not have access to a COVID-19 vaccine or may not be fully vaccinated before arriving on campus are required to receive a COVID-19 vaccine upon their arrival in Minnesota.
Students can continue to participate in orientation, classes and other activities, while wearing a mask, as they work toward becoming fully vaccinated.
Some students may have received a COVID-19 vaccine that is not currently authorized in the United States. In some circumstances, these students may be offered re-vaccination with an FDA-authorized vaccine.
